Alpine Home Health offers a broad spectrum of specialized healthcare services meeting the needs of adults and children in the comfort of their own home.
With a Physician Order We Offer:
- Skilled Nursing Services
- Before services are started; a Registered nurse assesses the patient’s medical history, psychosocial and physical status along with home environment and safety. After consulting the patient and his/her physician, the nurse will develop a plan of care with specific goals aimed at improving medical health, knowledge of medications and ability to stay at home. Each patient is assigned a Nurse Case Manager. The nurse case manager is responsible for monitoring the plan of care, coordinating all other skilled services and communicating with the patient, caregiver and physician.
Alpine Home Health offers specialized nursing to patient’s who require IV therapy, wound and ostomy care, or INR monitoring. - Private Duty Nursing
- For patients with long care needs, Alpine provides nurses and certified nursing assistants from a minimum of 3 hours to around the clock care.
- Physical Therapy
- Physical therapists provide restorative and rehabilitative services, improvement in range of motion, physical functions, gait training, fall prevention, education about the use of wheelchairs and walkers. Artificial limbs and pain management are common goals in a physical therapy plan of care. The physical therapist will regularly communicate with the Nurse Case Manger about the patient’s progress.
- Occupational Therapy
- Occupational therapists provide restorative and rehabilitative services. The occupational therapy goals often center on improving the patient’s ability to perform daily activities working towards improvement in bathing, grooming, dressing and eating.
- Speech Therapy
- Speech therapists help the patient with spoken, written and nonverbal communication. The speech therapist can also help improve the patient’s ability to swallow.
- Medical Social Services
- When a patient needs additional financial and community services, the social worker identifies what resources are available. The social worker can help the patient and caregiver obtain the services and will also assist the patient and their caregivers with long range planning and short term counseling.
- Certified Nursing Assistant
- Certified nurse assistants help with daily tasks such as bathing, grooming, dressing or assistance getting in and out of bed.
Private Pay Services
In order to meet the needs of individuals who would like to stay home but need some assistance, Alpine Home Health offers private pay services. These services include:
- Telemedicine and Telemonitoring
- Alpine Home Health offer a wide range of home monitoring for health and emergency needs. Our monitoring services range from the personal medication systems and emergency response systems to full health and medication monitoring. Alpine can monitor and manage complex medical conditions with the assistance of our simple to sophisticated systems such as:
- Voice Care, personal response systems.
- HomMed Sentry Monitor for preemptive care.
- MedPartner medication system.
- Learn more about Telemedicine and Telemonitoring.
- Personal Care
- Personal care services include assistance with bathing, dressing, hair care and toileting. Alpine also offers foot care programs to individuals with diabetic or other circulatory complications.
- Homemaking Service
- Some clients simply need help with housekeeping, laundry, shopping and meal preparation. Our trained staff will provide highly protective support care for these individuals.
- Medication Management
- Proper medication management is important to achieving good medical outcomes. Alpine will provide oversight for medication administration as well as education on medication side effects.
Without the patient’s permission, Alpine’s staff cannot discuss individual medical needs with anyone but the patient. However, a medical professional, caregiver, friend or clergy can call for information. Services are available 24 hours a day, 7 days a week.
